Whether you are a team coordinator, a committed team member, or just a investigating your options, we invite you to take advantage of these resources. Each item was created with you in mind. If you are looking for some specific information and can not find it here or elsewhere on the DM website please send us a quick note at info@DestinationMinistries.org and we will do what we can to answer your question and provide the information you are seeking.

The listed resources are primarily in an Adobe Acrobat PDF fill-in format. Some of the forms are also available in Microsoft Excel or Word format. To view, complete and print the PDF fill-in forms you'll need the freely available Adobe Reader software installed on your computer. Adobe Reader does not allow you to save your completed forms to disk. Therefore, be sure to print the applicable number of copies before closing the completed form.
